Fiscal Year 2013 HOME Homeowner Rehabilitation Program

HoJac Enterprises Incorporated will be accepting applications for the Homeowners Rehabilitation Program. This program offers a deferred forgivable loan to be used to repair dwellings suitable for rehabilitation that contain housing code deficiencies. Owners must provide the following documentation (Please, if you are submitting your application package by mail, Do NOT mail us your original important documents. We only want the original application form mailed. Please make sure that we only receive copies of your important documents.)

1. Social security cards for all household members
2. Picture identification of applicant
3. Deed or title to property showing applicant as current property owner
4. Proof of residency for a period of at least three (3) years (utility bills, etc.)
5. Proof of all sources of income to include paycheck stubs for the last eight (8) weeks for each employed household member, current Social Security or Supplemental Security Income (SSI) annual benefits statements, current year’s income tax return with W-2s, 1099s for each household member who filed a tax return
6. Name/mailing address of mortgage company and balance on mortgage, if you have a mortgage
7. Name/mailing address of your homeowner’s insurance policy provider with proof of current insurance policy
8. Proof of payment of property taxes and user’s fees up through the previous year
